In particular, alcohol is able to alter synaptic function by impacting multiple neurotransmitter systems, including 5-HT, DA, GABA, Glu, and ACh (Figure 2). The following paragraphs briefly summarize some of the main effects of alcohol on these neurotransmitter systems.

In general, studies using these approaches have demonstrated that the pattern of alcohol exposure (i.e., the frequency of withdrawals) appears to be as important as the cumulative alcohol dose in revealing alcohol’s negative reinforcing properties. Alcohol is excreted in urine, sweat and breath, but the main method of elimination from the body is by metabolism in the liver where it is converted to acetaldehyde and acetate. The rate at which alcohol is metabolised and the extent to which an individual is affected by a given dose of alcohol is highly variable from one individual to another. These individual differences affect drinking behaviour and the potential for alcohol-related harm and alcohol dependence. Also, the effects of alcohol vary in the same individual over time depending on several factors including whether food has been consumed, rate of drinking, nutritional status, environmental context and concurrent use of other psychoactive drugs. Therefore, it is very difficult to predict the effects of a given amount of alcohol both between individuals and within individuals over time.

However, a recent study [1] raises significant concerns about the accuracy of these diagnostic data. Yet, our misguided beliefs shape our perceptions, and our perceptions fuel our desires. Before we go further, let’s first discuss the difference between belief and reality. Most people don’t realize that what they perceive as reality is actually just a set of beliefs. The true reality of how the world operates is too massive for our human minds to comprehend. Therefore, we form sets of beliefs to interpret the reality around us based on our personal experiences, observations, and what is relevant to our needs. When your work, school, or family responsibilities start slipping, your drinking may have moved from being a social activity to a serious issue.

Given that alcoholism is a chronic relapsing disease, many alcohol-dependent people invariably experience multiple bouts of heavy drinking interspersed with periods of abstinence (i.e., withdrawal) of varying duration. A convergent body of preclinical and clinical evidence has demonstrated that a history of multiple detoxification/withdrawal experiences can result in increased sensitivity to the withdrawal syndrome—a process known as “kindling” (Becker and Littleton 1996; Becker 1998). For example, clinical studies have indicated that a history of multiple detoxifications increases a person’s susceptibility to more severe and medically complicated withdrawals in the future (e.g., Booth and Blow 1993). End-Stage – This final stage, known as the late stage, is described as total alcohol dependence, where you may experience uncontrollable alcohol consumption. Health conditions, like cardiovascular and liver diseases, may be caused or exasperated by your alcohol use, and death from alcohol poisoning or long-term effects of alcohol use is imminent if treatment is not sought.

This means they can be especially helpful to individuals at risk for relapse to drinking. Combined with medications and behavioral treatment provided by health care professionals, mutual-support groups can offer a valuable added layer of support. In female rats, alcohol has been shown to suppress the secretion of specific female reproductive hormones, thereby delaying the onset of puberty (see Dees et al. 2001 and Emanuele et al). Dees and colleagues (2000) found that immature female rhesus macaques exposed daily to alcohol (2 g/kg via nasogastric tube) exhibit lower levels of GH, FSH, LH, estradiol (E2), and IGF-1 (but not FSH or Leptin) compared with control subjects.

Current practice in treating AUD does not reflect the diversity of pharmacologic options that have potential to provide benefit, and guidance for clinicians is limited. Few medications are approved for treatment of AUD, and these have exhibited small and/or inconsistent effects in broad patient populations with diverse drinking patterns. The need for continued research into the treatment of this disease is evident in order to provide patients with more specific and effective options. This review describes the neurobiological mechanisms of AUD that are amenable to treatment and drug therapies that target pathophysiological conditions of AUD to reduce drinking. In addition, current literature on pharmacologic (both approved and non-approved) treatment options for AUD offered in the United States and elsewhere are reviewed.

Hazardous and harmful drinkers may respond to a brief intervention provided in primary care without requiring access to specialist treatment (NICE, 2010a). For others, their alcohol problems are overcome with the help of a mutual aid organisation, such as Alcoholics Anonymous (AA; see Section 2.10). Nevertheless, many will require access to specialist treatment by virtue of having more severe or chronic alcohol problems, or a higher level of complications of their drinking (for example, social isolation, psychiatric comorbidity and severe alcohol withdrawal).

The National Center for Drug Abuse Statistics says more than 20 million people over the age of 12 in the United States have substance use disorder. Addiction is considered “highly treatable.” But it can take a few tries for the therapy to be fully effective. One size does not fit all and a treatment approach that may work for one person may not work for another. Treatment can be outpatient and/or inpatient and be provided by specialty programs, therapists, and health care providers. Health care professionals use criteria from the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), to assess whether a person has AUD and to determine the severity, if the disorder is present.

For most people who are alcohol dependent the most appropriate goal in terms of alcohol consumption should be to aim for complete abstinence. With an increasing level of alcohol dependence a return to moderate or ‘controlled’ drinking becomes increasingly difficult (Edwards & Gross, 1976; Schuckit, 2009). Further, for people with significant psychiatric or physical comorbidity (for example, depressive disorder or alcoholic liver disease), abstinence is the appropriate goal. However, hazardous and harmful drinkers, and those with a low level of alcohol dependence, may be able to achieve a goal of moderate alcohol consumption (Raistrick et al., 2006). Where a client has a goal of moderation but the clinician believes there are considerable risks in doing so, the clinician should provide strong advice that abstinence is most appropriate but should not deny the client treatment if the advice is unheeded (Raistrick et al., 2006).

In general, offspring of parents with alcohol dependence are four times more likely to develop alcohol dependence. Evidence from genetic studies, particularly those in twins, has clearly demonstrated a genetic component to the risk of alcohol dependence. However, a meta-analysis of 50 family, twin and adoption studies showed the heritability of alcohol misuse to be at most 30 to 36% (Walters, 2002).

Those with moderate to severe alcohol use disorders generally require outside help to stop drinking. This could include detoxification, medical treatment, professional rehab or counseling, and/or self-help group support. Alcohol abuse was defined as a condition in which a person continues to drink despite recurrent social, interpersonal, health, or legal problems as a result of their alcohol use.

  6. Another method for assessing the reinforcing properties of alcohol is intracranial self-stimulation (ICSS).

Taken together, a substantial body of evidence suggests that changes in CRF function within the brain and neuroendocrine systems may influence motivation to resume alcohol self-administration either directly and/or by mediating withdrawal-related anxiety and stress/dysphoria responses. For example, in some brain regions, alcohol affects the expression of genes that encode components of the GABAA receptor. This has been demonstrated by changes in the subunit composition of the receptor in those regions, the most consistent of which are decreases in α1-and increases in α4-subunits (for a summary, see Biggio et al. 2007).

This latter group includes people who have been harmful drinkers or alcohol dependent in the past and who have stopped because of experiencing the harmful effects of alcohol. For more severe forms of AUD, in-patient residential treatment options are available. Residential treatment involves in-patient care at an alcohol-free residential facility with support staff and licensed counsellors, social workers, physicians, other allied health care professionals, and peers.

In the brain, in a single drinking episode, increasing levels of alcohol lead initially to stimulation (experienced as pleasure), excitement and talkativeness. At increasing concentrations alcohol causes sedation leading to sensations of relaxation, then later to slurred speech, unsteadiness, loss of coordination, incontinence, coma and ultimately death through alcohol poisoning, due to the sedation of the vital brain functions on breathing and circulation. Harmful alcohol use and dependence are relatively uncommon before the age of 15 years, but increase steeply to reach a peak in the early 20s, this being the period when alcohol use-disorders are most likely to begin. One US general population study found the prevalence of alcohol dependence to be 2% in 12- to 17-year-olds, rising to 12% in 18- to 20–year-olds (Grant et al., 2004a).

Tolerance refers to a decrease in the reinforcing efficacy of drugs following repeated exposures. Once neuroadaptation has occurred, removal of alcohol from the organism leads to a withdrawal syndrome. The positive reinforcing effects of alcohol generally are accepted as important motivating factors in alcohol-drinking behavior in the early stages of alcohol use and abuse.

Therefore, it is helpful from a clinical perspective to subdivide dependence into categories of mild, moderate and severe. People with mild dependence (those scoring 15 or less on the Severity of Alcohol Dependence Questionnaire [SADQ]) usually do not need assisted alcohol withdrawal. People with moderate dependence (with an SADQ score of between 15 and 30) usually need assisted alcohol withdrawal, which can typically be managed in a community setting unless there are other risks. People who are severely alcohol dependent (with an SADQ score of 31 or more) will need assisted alcohol withdrawal, typically in an inpatient or residential setting.

Homeless people who misuse alcohol have particular difficulties in engaging mainstream alcohol services, often due to difficulties in attending planned appointments. Al-anon uses the same 12 steps as AA with some modifications and is focused on meeting the needs of friends and family members of alcoholics. Again, meetings are widely available and provide helpful support beyond what can be provided by specialist treatment services.
